The First's hell dimension was a hell dimension created and ruled by the First Evil. It had complete control over the dimension and knew everything that happened within it. In-game, it appeared as a dilapidated and nearly deserted version of Sunnydale reminiscent of the Wishverse, where vampires and demons apparently reigned supreme.

In the twelfth century, Cassandra Rayne, that time's Champion, battled the First with Hope's Dagger, but was defeated before she could deal the killing blow. Though she was rendered immortal by Hope's Dagger, the First nevertheless dismembered Cassandra's body and scattered its pieces all over its world.

Centuries later, Cassandra's descendant, Ethan Rayne, made a bargain with the First for power, and the two set up a contest. If Ethan won, he would obtain the power he had longed for for so long, but if the First won, then Ethan would suffer for eternity as the whipping boy for the lowest toadies in the First's dimension.

The First and Ethan each chose five champions to face one another in combat. For Ethan's part, he chose his old enemies, the Scooby Gang; namely Buffy Summers, Faith Lehane, Willow Rosenberg, Xander Harris, and Spike. The five chosen by Ethan were unwillingly transported to the First's dimension to partake in the contest, but, upon hearing of the legend of Cassandra and Hope's Dagger from their old ally Sid, who had been captured and forcibly imprisoned by the First after his death, they took it upon themselves to reassemble and resurrect Cassandra and obtain Hope's Dagger, which Buffy used to defeat Ethan after he was empowered by the First. Afterwards, the Scoobies were transported back to their home dimension, while Ethan was apparently left behind.
